Purchase Orders

Midwest Pro Sound and Lighting accepts purchase orders from U.S. Schools, and US Federal, State, County and City government agencies.

For Purchase Order use:

1. 1. Contact Midwest Pro Sound and Lighting in order to receive a shipping quote for your order.  This shipping total MUST be included as a line item on your Purchase Order.
2. 2. Fax a formal P.O. with a cover letter on your official letterhead to 773-975-4267, attn: P.O. Dept.  Be sure to include contact information, including an email address, in order to receive your shipping confirmation and tracking number.
3. 3. If you need additional information or have any questions concerning your order or the process, please email us at info@midweststereo.com or call 800-975-1613 and ask for the P.O Dept.

For fastest service, be sure to include the following information with your faxed Purchase Order:

• Your P.O. Number
• Organization Name
• Contact Name
• Contact Phone/Email
• Billing Address
• Billing Contact Phone/Email
• Shipping Address
• Tax Exempt Form (if applicable)
• Item List and Quantity
